Rainier Lifestyle Medicine PLLC ("we") follows Washington's My Health My Data Act. This policy explains the consumer health data we collect through this website and your rights.
We collect one item: the email address you enter in the "Notify Me" form. Because it shows interest in a health care practice, Washington law treats it as consumer health data. We use it only to send one email when the practice opens.
You give it to us directly through the form or by emailing us.
We do not sell or share your email. Two service providers process it for us under our instructions: Netlify, Inc., which hosts this website and stores form submissions, and Northwest Registered Agent, LLC, which hosts our business email and delivers the notice to our inbox. We have no affiliates that receive this data.
You can ask us to confirm whether we hold your email, send you a copy, delete it, or withdraw your consent at any time. Email info@rainierlifestylemedicine.com with your request. We will respond within 45 days. If we deny your request, you can appeal by replying to our answer, and we will reply within 45 days. If your appeal is denied, you can contact the Washington State Attorney General at atg.wa.gov.
We will post any change to this policy on this page with a new effective date.
